COMS Pamphlet
I've created a mostly-complete pamphlet for your practice. Due to restrictions on what I can upload, I could only upload PNGs of a selection of the pages here. In total it's 22 pages, using the language from the links you've provided. I've also left a page blank at the end for you to insert a diagram for the surgery swag pack. It's in PowerPoint, so it's very easy for anyone to edit the text, look, and layout, but it can easily be converted to an Epub, which seems to be the format you prefer.

Very good work! The content is excellent. I will provide feedback on content, but your are mostly spot on. As mentioned in the description, we would also like to include media files such as short video tips. I assume these can be integrated easily. I would also recommend inserting stock images of happy people. These can be general scenes, action shots or happy people in medical environment. Just to be sure, there are no copyright restrictions on the current content? Again, great job!
