Directory site creation - written in PHP (Zend framework)

We're looking to develop a new directory site with the following functionality:
- Directory listing
- Scheduling system to book appointments
- Customer backend for customers to update profile/availability
We'd like it written in PHP (Zend framework) and would be happy for you to either quote on a white site (no graphics or layout), or quote frontend also based on our graphics provided.
We would like an example of a directory site you have personally completed. This is an essential component of your bid.
We would also like you to estimate how long a site with the above functionality would take to code, and a rough quote for the site. We would also like you to provide your hourly rate.
