Analyze some Data

I am looking for someone to accurately enter data from a PDF file to an excel spreadsheet. The PDF file has 177 pages in total and around 16,000 total entries. Each entry comes from an employment directory, and contains the person's name, phone number, and other relevant contact information.

Please see the attached picture. The red square highlights 1 of the 16,000 or so entries. All this information would have to be accurately typed into an excel spreadsheet, with separate columns for each item type (i.e. name, phone number, fax number, email, etc.).
