Design and Create My Magazine Ad

Hi,
I'm looking to do an ad in health/fitness-related magazine and am looking for help in designing/creating my ad. My ad will be 2.125" x 3" and 4-color. All instructions/specs will be forwarded to awarded freelancer. I need someone creative with a marketing vision, although I have somewhat of an idea of what I'd like on there. This project needs to be complete IMMEDIATELY as my deadline is approaching. My product is motivational bracelet system which I will explain as well to winner.
Aside from the ad design which must be done as per magazine requirements, I need same person to do a very simple UPC sticker for me. I already have the size/design of sticker from previous collection. I just need this person to change the information and code for the already existing design. This will take less than 5 minutes ;).
Please contact me with samples of any related artwork you've done.
Please don't bid higher than $40... I'd like to stay at the $30 range if possible as I am a struggling start-up.
