The web form should be universal and highly configurable.
-Install as a page on a server or standalone
-Should store all the requests on a local or network drive in the form of a SQL or MSACCESS (SQL preferred)
-Should dispense request numbers.
-Should allow batch upload of values to select from an excel or flat file by field and scan a local or network folder for the file to upload for a certain field.
-Should allow Form Administrator to input fields.

-admin console should allow for request lookup by number or keyword.
