I have a fairly simple project. I want to list a few things about a disease on a webform. Depending on whether a person checks 'yes' next to each parameter, i want to the website to suggest a plan of action. I already have the plan of action already written out for each combination of checkmarks. I want it to autodisplay the results at the bottom of the page based on the checkmarks.
I want to the design of the form to be simple. I want it to display mainly for mobile phones, so let it be mobile friendly.

Should be very simple for a person with knowledge of PHP, javascript, and Ajax if needed.
