Adobe Muse Form Widget

I am building a website using Adobe Muse. I need a form built with

• 5 text fields

• one single selection radio field

• about 60 questions using an input slider

I'll be replicating the questions here: [url removed, login to view]

The form will have to work on desktops and mobile devices on the Adobe Muse platform.

Solutions to this project may be html code or possibly a Muse widget.

All solutions will need to send results to a mysql databse and an emails to both the form submitter and the website administrator.
