Create Front/Back End of a SaaS Application (sketches provided)

The SaaS application requires 5 simple sections (most cases its only 1 page):

1) Signup/Login page: includes "Forgot Username/Password" and a pricing page should be created with 3 options. Payment should be done via PayPal.

2) Homepage: alphabetical hyperlinks to each "client" profile. Users also have the option to import a file here using a CSV template with fields from #s 3 and 5 below.

3) Client Intake Form: contains fields for dropdown and checkbox options along with notes sections.

4) Log entry form: like #3 above only these results will display in a table contained in #5 below.

5) Client profile: includes a table with entries created from #4 and displays the info from #3 above.

SaaS Details:

- Numbers 2, 3, and 5 above will have a universal search box that will allow you to search any client profile by name or any other field (perhaps using the "Chosen" plugin).

-Query the database to on the front end to search client profiles by various filters from #5 above.

- Administration page to create, activate, deactivate accounts

- CSV template to import data

Project Details:

- References: please provide links to work you've completed in the past. If chosen for this project, I will ask for the contact details of those clients to confirm.

Coding Language and Rationale: tell me what you will use and why for both the front and back-end

- Delivery of Source Code: one of the final milestones will be delivery of the source code. Only then will the bulk of payment be made.

- Server: this should be setup in the cloud. Confirm what you think the best option is and I can setup an account on that provider's service for you to use.
