Very simple .ico job that needs some attention

We have PSD's for Windows executable icon. We need two icons to be 256*256 size 32 bits in Windows .ico format. This is for the exe to be seen on Windows explorer.
I thought this would be something simple yet two coders (they are actually web designers) FAILED in delivering this! Really strange, so nowadays when somebody says "icon", it is "web icon" (GIF, PNG)! So I require the designer to contact me on Skype. The reason I do not do this myself is we do not have PS installed and do not like GFX works.
