Two Small Documents for LLC to Bring in Small New Investment

I have an LLC (Illinois, USA) with an existing, original, unmodified operating agreement from 2014 and also 9 members. This operating agreement is about 25 pages. Three of the existing members will be investing in a current follow-on financing. I need two simple documents, where are probably 1-2 pages each. The first document is need an amendment to the Operating Agreement authorizing the sale of additional interests and signed by at least 5 of the 9 members. The exact value of the additional interests being authorized is 3.75%. The second document is an agreement between the LLC and the three investors (i.e. 3 of the 9 members who are participating), where they each contribute $15 K and receive an additional 1.25% interest. I have a detailed "before" and "after" capitalization table to share which explains what everyone has agreed to, and I just need legal documents for signatures.
