Need a Fillable PDF With Conditional Logic

Hi - I have a static PDF that I would like converted to be a fillable PDF.
IMPORTANT NOTE: Please look at the attached PDF in the section titled Menu Selection on the second page. It is important to understand this requirement. Here is how it should work:
1) User chooses one of the 3 options ($45, $55 or $65) from a radio group.
2) Depending on which radio group is selected, there should appear a list of choices as follows:
2a) First Course: A list of 4-5 items with checkboxes (can select a maximum of 3)
2b) Second Course: A list of 4-5 items with checkboxes (can select a maximum of 3)
2c) Main Course: A list of 4-5 items with checkboxes (can select a maximum of 3)
2d) Dessert: A list of 4-5 items with checkboxes (can select a maximum of 3)
That's the biggest bit in this project. The rest of the form will be pretty much as it is visible on the page.
Please let me know your best estimate for this. It may be higher or lower than the budgeted amount; I just have to enter something to post the job.
