Create & Design Plumbing Referral Website

I am trying to recreate the following site:

I want the site to function similar to:

I need to hire someone to create the initial page complete with a new logo, and create a few extra pages. I want all the states to be listed on the left side of the page.

The winning bidder will:

1. create and design the [url removed, login to view] main page complete with state listing, logo, and extra links.

2. create (1) state folder - for example: New York with (2) pages for counties. example: Nassau County & Suffolk County

3. create a payment page (will link from paypal)

4. place 3 ficticious listings on the 2 county pages complete with fake company name, address, logo, website, etc.

5. the site will be SEO and search engine friendly.
