Form to mysql with email notification

I have 2 forms in Microsoft Word that I have attached to this posting that I need to have converted to html. When a user submits the form it should be recorded in a mysql database and a email notification sent. The form needs a simple captcha that is very easy to read. Everything should be on one page including forms and submission. My budget is $30.
