I need this drop down converted into a magento module: [url removed, login to view]

All required code is provided there, it just needs to be converted to a magento module.

The only addition/changes you need to make is add one button at the button that redirects to the corresponding page associated with that country/state/city that is located in the SQL database. For example if you selected: India > Punjab > Amritsar it would go to [url removed, login to view] which is located in the database-- very simple

This should be very easy if you are familiar with magento.
