Customize form page in wordpress -- 2

Task 1

Information -

I have a custom built form page with superforms in

wordpress Parent/Student details form.

Here is the final form created.

Task to do -

Once the form is submitted the entry to be shown in a page for the user with an edit option. So the user can see the info and update it whenever he/she likes.

Task 2

Information -

In the above mentioned form user submits info about participating students and their grade/Class

Courses will be divided into 4 grade/Standard

Grade 1 to 3

Grade 4 to 6

Grade 7 to 9

Grade 10 to 12

Task to do -

I want dropdown field to be added on the cart page near to each product [login to view URL] see the attached image for understanding better. The information of the dropdown field will be pulled from the submitted parent/ student form.
