Database with Web Based front End Management

Database backend (Currently in Microsoft SQL 2012 Format, may need to convert to mysql format to use with the front end)
Web Based Front End:
Has to show data from database in the following way:
Database is made of of people, each with a set of attributes attached to them. (Name/.address/phone/email/pollingstation) etc.
Example, You should be able search an individual based on their address, polling number assigned to them etc, all this info is in the database.
Secondly, the web based part would need to have permission based login, so only certain users can edit certain fields etc on the user entry.
Third, a field of Yes or No for each person in the DB is required, and we can either check yes or no in the front end, and a count should be kept of yes / no's.
Statistics should be also shown in the front end, general things like: Total users, total users based on polling area / address, total yes/nos etc
This is fairly straight forward, a database with a web based front end pulling a couple of sql queries with the ability to edit etc based on user permissions.
