Simple one page Android App to query a DB - No graphic design requiered

Hi!
I have a simple database with 4 tables where I store districts, cities, streets and houses (just the street’s number). On the "house" table I store a few fields to check if some (6) door to door mailing campaings have been delivered or not (Y/N/?) and their priority (from 1 (max) to 127).
An aditional table will store user's id and user's API keys.
I just need a simple one page Android APP where I can search over these tables on the top, and show the related info below, using dependant text fields with autocomplete (I'll search “district -> city -> street -> number” and it will provide the info stored about this address).
In some cases a button will appear to update some data on the houses table("Set Priority to 1" for the selected Campaign).
- you will provide a simple Rest API to deal with the queries
- requests to the API will be identified with 'user_API_key' in the headers (so I'll use https)
(or even better identified with the 'user_id' and signed with his 'user_API_key')
- responsive app can be made using jQuery mobile & any free plugin you could need
- I don't need any graphic design or logo (I will provide a simple logo in SVG and PNG formats)
- I don't need the App to be available on Play Store, it's just for me
- I don't need any add, banner, whatever to monetize the app, again it's just for private use
- I will provide a database to test and full specifications to avoid wasting your time where possible.

PS: Please note that:
- I also need a webpage (could be made with jQuery UI) with the same design & functionality (to be hosted on an external server SO this will use the Rest API as well to fetch the data from the DB server). Please message me with another estimation price for this.
- If this APP proves to help me in my daily work I will need to implement a few improvements very soon so I’ll pick up the same coder again.
