Service entry with CSV export

Allow users to select mutiple service items from the screen and export the corresponding service code to CSV.

## Deliverables

Need an application that will have the account number service date and the set of services performed . There are 12 types of services in one group and around 8 types of services in another group. Each service is associated with a service code. The end user will enter the account number and the service date and select the service items from the screen by clicking on the checkbox. ( they should also be able to enter the service code of item in case it is not in the items to be selected) . They will do the same for multiple account numbers in a batch. If they select atleast one of the items in group 1 , it should automatically add a service code (10250) .Once finished they should be able to click on export and it should create a csv file with the account number, service date, service code (one line for each service code) .

Need full source code. Can develop in C# or vb.net with sql express or access database
