Coder must complete the coding so that the attached sheet will work as described.
1. Entity will get values from another sheet that will continue a list of valid entities
2. The column "Type of Healthcare Worker" will be the only values visable until the user selects a value in the dropdown .
Once the user selects a value the rest on the controls in the row will become visable.
When the user clicks on Submit & Reload - All rows with data will be entered into an MSAcess Database table and the form will be cleared for more records to be entered (Each record will contain 13 fields.)
When the user clicks on Submit & Close - All rows with data will be entered into an MSAcess Database table and the form will close (Each record will contain 13 fields.)
