Convert MS ACCESS database to MYSQL database them import into testing application

The total project will consist of 8 database conversions from Microsoft Access to mysql then inserted to work in an online php testing application called phptest. You can see documentation here (I think they are re-doing the site [url removed, login to view] You can download the zip file I have attached that has a copy of the testing application in it. Also in the attached zip file is one database that will ultimately be used in this project, a php script written by the author of the testing program to help assist with the insertion/conversion. Once I assure all the questions are inserted properly I will accept your bid Each database can have as many as 1700 question (over estimate) and as low as 500 questions Categories to be used are Human body,Airway Respiratory Emergencies,Allergies,Behavioral Emergencies,Soft Tissue Injuries,Infants And Children,Rescue Breathing/ CPR,Well Being of the emt-b,Baseline vital signs,Patient Assessment- Plus,Cardiac Emergencies,Poisoning and Overdose,Musculoskeletal Injuries,Obstetrics & Gynecology,Ambulance Operations,Medical, Legal and Ethical Issues,,Lifting and moving patients,General Pharmacology,Diabetes and A.M.S.,Environmental Emergencies,Bleeding and Shock,Injuries Head \ Spine,Gaining Access and Overviews Things that are to be left out Custom test Terminology Take the entire test (if it even exists) Introduction Page or Book Reference The only data that should be transferred over as far as I know is the question, the answer code and the four possible answers. All of the test will be multiple choice

## Deliverables

1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.

2) Deliverables must be in ready-to-run condition, as follows? (depending on the nature? of the deliverables):

a)? For web sites or? other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.

b) For all others including desktop software or software the buyer intends to distribute: A software? installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.

3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
