Build Medical Mission Site to Generate Graphs from Surveys

We are a non-profit organization.? This site was created first in 2005 and the framework is outdated.?

Site Scenario:A user comes to the site and either signs in or creates a new profile. The profiles include: (require)

Name:

Email:

Role: (nurse, doctor, mission director, patient, visitor)

Mission: currently there are 5 missions registered, these would be available in a drop-down menu along with the option to add a new mission. If the option to add new mission is chosen another page/request form would open for all pertinent details on the new mission. This new mission must then automatically be included in the drop down menu for users to choose in the future because these surveys on each mission need to update reports and graphs dynamically with each survey completed.

Once the user is logged in they choose to complete the surveys begin for mission selected or the new mission created or view reports on missions that have been added earlier. If they want to move on and print a report or email a report on any or all missions, this is a vital need.

This is a site created to allow medical missions' to be critiqued by doctors, nurses, directors etc. The surveys generate a line graph and pie chart based on the answers given. One mission/survey can be accessed by several users adjusting the results. The results should be printable or be able to be sent by email.

What we have been given is the compressed project archive, as well as an older version of the Lampshade framework you'll need to run it. A general technical guide for Lampshade can be found here:

The main feature and what has been requested is that the surveys are accessible and that they can be queried to make the various pie charts and line graphs? previously installed. If you know of a better way to reach to same end we are open to your suggestions.
