birthday reminder wordpress plugin

Is required a WP plugin that give in front-end the possibility to insert 5 birthday dates (with javascript calendar) of 5 friends (input for friends name). In this page is inserted also user email address and is required a captcha, and javascript check for all fields, A week early date of birthday of each of them a mail is sent to the user, to remind him that date. In back end a small configuration panel give the possibility to configure - mail content to send to the users - list all users registered (with edit/remove functions) To create registration page will be used a shorttag (ex: [friendbirthday] ) in post content. Tables involved in the plugin will be created during plugin installation.
