I have a completed database design and form running on Access. I need to have this form which is based on a single query converted to run from the interenet. The form was generated from the form designer and has multiple calculated fields and combo box selection lists. I need the form converted and a front window to capture a two fields which will be used to query the corresponding record from the access database. The user can only see their record and can edit and save changes.
## Deliverables
1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Installation package that will install the software (in ready-to-run condition) on the platform(s) specified in this bid request.
3) Exclusive and complete copyrights to all work purchased. (No GPL, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site).
