CSI Web Page Form

Need an Estimate Request Form for a Web site. Has three sections.

Opens with text.

First section has10 fields (3 fields are required)

Second section - 5 fields plus 7 topics w/related boxes to check off (19 boxes total) and one field for comments (need to include character limit.

Third section has 25 topic related check boxes, Add'l info box (w/character limit), and a Scope of Work box (w/character limit).

An additional field will be needed to allow a customer to attach a blueprint, spec sheet, etc. This will include a Browser button to search their desktop and attach a file.

**Update - This bid is for HTML coding only w/CGI Scripting that allows the form to be sent to an e-mail address. Attachment forms must be in pdf, .jpg, or a zip file.** **No php or asp coding is necessary at this time. A Completion page and Error page have already been created once the request is filled out.**

## Deliverables

1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.

2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):

a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.

b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.

3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).

## Platform

This is a Unix server. The form needs to be viewable by all browsers - IE, NetScape, Foxfire, Google, Yahoo, etc.
