XWIKI Expert Required

I'm having trouble with a customize implementation of XWIKI:

Sometimes, when an item is created platform trows an error saying that there is another item with the same identifier, but there is not, and either case there is another item or not the new item is created.

Web site is available over the Internet and linux server can be accessed.
