Contest Form with Facebook Connect and “Like-Page” Validation

# Quick Introduction

Please read this and the attached document ***completely*** before posting a bid.

We are looking for someone to develop a simple PHP contest form. The goal of this contest is to gain more fans for a specific Facebook page that we own. To achieve this goal we are giving away a free gadget (Apple iPad 2) to whoever wins the contest.

There will be two multiple-choice questions (with radio buttons) and one tie-breaker question (a numerical text field).
