Create Job Application Form For My Website

Hey everyone, need someone to make a form on my website so people can apply for a job. I doubt anyone will apply or use it at this point, but I want it setup so its ready to go for later in the future.
The current website is html & not mobile ready, but it will be soon. So please make the form in some sort of responsive style that will easily adapt & still look good when we switch over to the mobile site.
I will pretty much need to collect all of this info listed in this example job application form: [login to view URL]
Also I had this one made a long time ago, you can tear it apart and salvage the code if you like. Not sure if it would be easier to do that, or simply start over from scratch. I'd imagine this one would need some updates / upgrading. This was from my other website: [login to view URL]
When users push the submit button, I'd like to receive all the info & attachments in an email to a specific email address. As for the look of the admin email, just something clean and readable. Maybe a little nicer than just plain text. All the user info would need to be transferred securely too.
Will need the form to have a look and style that goes well with our current website. Please test it out and make sure it works first. One or two rounds of corrections may be needed. Not much of a budget for this one, but please bid accordingly. Looking for someone to do great work & available for more web projects as well. We like finding people for the long run. by using web development tool i will make a page in which we interact with customer asking some details about job and he/she fulfill the form and enrolled in job applicants.
