i need to create an excel spreadsheet to help employees writting a service order form using a contract that is already excisting, i already have an example of what i need.
i just need it to be done with the new contract we have
attached 3 excel sheets
1st is the actual service order form that we normally write depending on service we provide using the contact we have with the customer.
2nd its the contract itself with all the services and charges.
3rd is a example that was done before for another contract.

(basically the reason i want this input sheet to be done is to make it easier to write the service order with less interferance from the employees to make it easier and minimize mistakes.)
i need an output as a printable version of a service order as the one i attached as exmaple. (details will be discussed later)
