We have a PDF form that we send to new employees to obtain employee details. The form was created automatically in Adobe Acrobat from an imported Word document that I created. I would like the layout of the form adjusted so that it is more professional and with drop down selection options where possible. I require two versions of the form 1) with Proximity Logo and 2) with Proximity Legal Logo. The logos are available in other formats if required. Our website address ([login to view URL]) should be added to the footer of the form. If there is an easy way to have this as a secure web form, in additional to a pdf form that can be completed electronically or as a hardcopy, that would be a bonus.

Hello and thank you for a chance to offer you my services, I can design pdf form for you and I would recommend to reduce the size of your form to 2 page. Also, you need to be aware that drop-down menus are great if you want to use your form as digital form only. But if you would ever consider to share printed versions of this form then you would prefer to use check box selection instead of drop-downs, so users can see available options on paper.
