Design a Brochure

I would like an attractive PDF to be created URGENTLY as part of an opt in Facebook marketing campaign which will be running of a Lead Page. The purpose of the PDF is for participants of my 7 day wellness challenge (which will be delivered via an email sequence) to use as a journal to record their answers from each task given on each day of the challenge. I want the PDF to be inspiring, attractive and to match the look and feel of my leadpage, including the banner colours of the leadpage. I also want it branded with my logo and pictures of myself. As you will see on the Leadpage - there is a theme of blossoms in the background and in the photo of myself - if possible - is the background of the PDF able to have blossoms faintly appearing in it? [login to view URL] I want the PDF to have a banner picture of myself up the top (which i will supply) and have my logo, website and facebook page on it. There are 7 action steps - 1 for each day of the challenge Please see the Word document for further instructions. I would like have a page allocated for each action step - with lines put in so the answer can be written down. at the end of each action step i have a quote that needs to be put in. I would also like at the end a section for "further notes". thank you!
