This should be a very simple project for anyone with Adobe Acrobat or compatible software. I need fillable form fields added to the attached PDF document. The position, font, size and justification for the fields are indicated by the bordered boxes. The fillable fields should be the same size as the bordered boxes, but should be borderless, with margins set to 0 on all sides. If possible, I would also like to improve the quality of the graphic in the upper left corner of the document. For some reason when I convert the document from Word to PDF, the graphic gets pixelated somewhat. I can live with it, but preference will be given to someone who can solve this problem as well. Original graphic is also attached.
