complaint tracking web site

I am building a web application that allows my employees to fill out forms for tracking customer complaints. The forms contain information on customers such as customer name, address, contact person, phone number, problem or issue notes, AND relevant electronic files (for example, PDF, Word, or Excel documents). The forms also contain information on how much time is used to address the complaints. There are 3 forms: 1. FORM S - contains about 30 fields 2. FORM F - identical to FORM S, but only accessible to certain users 3. Phone log -this form records content of phone conversation. A FORM S or F can contain several phone logs. The application also needs to have reports, e.g., weekly/monthly reports of how much time and money were devoted to FORM S and FORM F complaints. Another feature is a calendar (printable) showing when actions are required for the forms filled out that month. The forms contain a field called "action date," and by assigning each form a different color and displaying the information on the calendar, I will have a rough idea know how much work needs to be done this week/month just by glancing at the calendar. Manual entry should also be possible. The web application should use J2EE and MySQL database. The applications should also be well-designed, robust, and easy to modify (for example, adding/removing a field). ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Installation package that will install the software (in ready-to-run condition) on the platform(s) specified in this bid request. 3) Complete ownership and distribution copyrights to all work purchased. Buyer will receive exclusive and complete copyrights to all work purchased.
