Create a Survey Wordpress Plugin

We need a Wordpress Plugin to allow the admin user to create surveys (in Wordpress admin system). Here is the tree structure of the survey we want: Survey: - Field: Title (text) - Date created (save it automatically after insert) - ID (create it automatically on insert) - Questions (Unlimited List): - Question - Field: Name (text) - Responses (Unlimited List): - Field: Title (text) - Field: Detail (text) - Field: Weight (select box - 0 to 10) We will also need a screen to list the registered surveys and a way to edit and remove them.
