Connect Opt In Page to Word Press Blog (repost)(repost)

I have an opt in page at my website. I need this opt in page connected to a new Word Press blog where visitors will be taken to when they opt in. On the front page of the blog you will place a PDF that I will give you. The header of the blog must have the logo of my website, which I will send you. Also, you'll make some adjustments with the color scheme and appearance, etc. of the blog. Also, on the opt in page I want the font adjusted, and also the opt in box on that page.
