I need an online assessment form where participants answer a series of questions with the results of their answers tallied, and a final assessment emailed to them in PDF format.? There are no right or wrong answers (only answers that are preferable to others). Answers they choose add up to a score, which then equates to a piece of text that they'll read dependent on that score. In other words, each answer option is weighted, and the scores are added up, and then a message which depends on the score is served up in PDF format (which we will provide) sent to them via email. This script will need to be customizable so that we can change questions and answers for future similar assessments, to produce the PDF we choose to correlate within the range of weighted responses.
