Access Form Needed

I currently have a Word form that gets filled out, saved by the originator, emailed to the approver (for approval), emailed out again to a distribution list in outlook and save in an Access database. I now want to have the existing form created right in Access, but have the same functionality it did in Word. When the database opens, I would like two have 2 options, either 1). Fill out a hold form or 2). Search the Hold Database. All the fields in the Word form "connect" to the fields in the database. I couldn't get them all to work, but that is what is supposed to happen. I'm attaching all the files, including a copy of the database. I would like all the functionality that the Word form has in the newly created Access form. I even want the forms to look identical, if possible. Also, there are 2 save buttons on the word form. A Save and an Save To Database. Anyone can save the form, but there are only certain people who should be able to save it to the database (my team, in which I can provide names later). I don't care if the form is newly created in Access or if you'd prefer to use the code from the Word form. But again, some of the fields do not work in the Word form. Lastly, almost all fields should be mandatory, which I am unsure if that works in the Word form or not. We can go through those later, as well. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Installation package that will install the software (in ready-to-run condition) on the platform(s) specified in this bid request. 3) Exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement). 4). Well written instructions within the code so I can change things around and know what they mean if I ever need to. ## Platform Windows 2000/Access 97/Word 2000 (This is the only version of Access we are allowed to use).
