Create a new dynamic ASPX page for a website

I have an existing website. I need a coder to create a new dynamic ASPX page with connection to a MSSQL base database. The information added in the ASPX page will be saved in the existing fields of the tables in my database. Some of the fields should be created. You will create the page along with taking care of the page layout. You should also provide the SQL script to add fields to the existing tables or create new tables if need be. The labels, text fields and other elements will be provided. I need this quickly and you should be able to deliver this in 2 days for tests. I have added the page layout and the mappings to the database in the attached file. Please look at this file before bidding. You should consider some extra time for stuff that might come up and is not disclosed in this statement of work. Please consider double the time you need to complete this project. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased.
