Build a behavioral log in Google form/sheets/docs

I work with a residential treatment center that treats minors. I would like to transition their behavioral log to Google suite. The log should work like this. Staff while logged in to their google account (provided by the treatment center) would open a google form to make a log entry. The form would include the name of the resident the entry is concerning (if this could be done with a dropdown that would be great), a rating on a five point scale (1=calm/happy 5=Out of control) checkboxes for any flags that apply (attention medical, attention mental health, run-risk, suspicious behavior, etc.), a place for the entry of text (long form... one sentence to multiple paragraphs. When submitted the entry is placed in a google sheet. So far that is the easy part (other than the dropdown of names). I have already created this form that logs that info to a spreadsheet. This second part is where I really need help. Staff should also be able to read the log in a convenient way without looking through months of entries. They should be able to search by date or date range and see the entries for those dates. They should also be able to search for all entries about a particular resident. And they should be able to search for all entries with a 5 point scale rating above a 3, for example. While viewing the log, staff should not be exposed to the "raw data" or be forced to do formatting to get the data into a readable format, nor should they be able to change the data while viewing it. Security is critical. Only authorized users should be able to enter or view data.
