We are looking to create a Facebook app that sits as a tab on a Facebook page, allowing people to make a service request and provide all information required about that service request. On completion, the request will be emailed to a designated email address. This "app" or "tab" will need to be easily replicated so that we can apply it to multiple pages, each with it's own receiving email address. We will also need to be able to turn this facility on and off remotely without having admin access to the page. Image examples of what the page should look like can be provided to applicants on request.
