1- Fill a form (First Name, Last Name, DOB, Phone and Email). 2- Choose either to take a Picture (@72 dpi) or Video (Max 5 sec). 3- Review the Picture (See the picture in a preloaded custom border which would be loaded before the app is used ) or Video, if accepted go to next step. If not have the ability to retake. 4- Hit a "Finish" button which will send the form to a specific database, send an email to a specific email address, send the picture/video to the phone number as a text. 5- App goes back to Step 1.
