Write an iPad application

Form 1. Need a single form to get data into 3 SQL Server tables. Each individual using the iPad form will use his/her own generated sequence numbers. The form will have about 10 drop downs being fed from different SQL tables. There would be starting and stop time for the service done by the individual using the form and based on the start and stop time, The application would check if the data is overlapping with other users using forms on their iPads.
The application would provide entries to be selected from the drop downs as well as edit capabilities in case the data is not in the dropdown.
Form 2. Would have capabilty to insert upto 4 rows into a SQL Table, storing about 15 fields.
Form 1 and 2 would be linked by the Order ID ( Sequence Number ) as well as on 3 other fields coming out of Form 1. Example ( Customer ID, Terminal ID, Driver ID )
A demo would be given for the existing front end application, to help the developer have an idea.
