I'll need some more details before I can be firm on this bid. SurveyMonkey is a large and complex site with data mining and reports. We'll need to get into the specifics of what exactly you want. I also need some information about the technical setup for the site: 1) Will I have access to the SQL server, or are you expecting a .sql file to deploy to your server. 2) Is the server hosted in a load balanced environment, or on a single server 3) What version of Sql Server and IIS do you use 4) Is it acceptable to do this in MVC4? If you need the source to be buildable on visual studio 2012, MVC3 may be a better fit. 5) Although I'm a skilled programmer, I am not a graphics designer. Is there any content/images provided, or is a boostrap based layout acceptable? (I tend to lean towards the Microsoft/modern UI style, so if you'd prefer something different I'd need to know that too) 6) Does your company have an existing authorization system, or would this web app have its own internal authorization?
