Program needed that can read and interpret real time numbers that are constantly changing.

In need of a mini-program that can: 1) read/identify a number anywhere on screen/machine; 2) interpret what the exact number is (e.g. 1.29458, 1.29457, 1.29458) 3) extrapolate that number onto a layered bar graph based on time (e.g. 5 mins bar graphs)
