Simple Registration Page for Community Site

I need a registration page implemented for a community site. The spec is attached as a zip file. The spec is not complete, but should give you enough idea of the scope so that you can place a bid.
I will also provide a text field definitions (max length, what is valid, etc), db schema and mock objects which you can code against. These mock objects represent our internal API. I will provide this before I accept the bid, so you can adjust your bid amount if you feel that the work is less or more than you expected.
Coders must also be willing to sign a Services Agreement which includes agreements to confidentiality and non-disclosure. I will provide this before accepting the bid so coders can take a look at it first.
This should be implemented as a single servlet, which handles all operations. The operation should be passed in as a parameter:
/signupServlet?op=register
The display of pages should be implemented as a JSP. You should then forward to the appropriate JSP in the servlet code.
There will need to be an email that is sent out. The email text should be generated using Velocity.
## Deliverables
1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):
a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. This includes a [login to view URL] to build a jar and a war file that can be dropped into any servlet environment.
3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
4) Signed and delivered Services Agreement that is to be provided later by us.
