oscommerce extra check out options

We have an oscommerce ecommerce store. We sell all ocassion cards, on check out we need a #1)text box that the buyer can enter who the card is from, #2) a text box that the buyer can enter who the card is to, this would just be the name of a person or family example "George" #3) a comments section that the buyer can enter text in if they want to add their own words on the card, and #4) a way to upload an image or two with the order in case they want to use their images on the card instead of the default image we provide. #5) a section the buyer can put the physical address to the person they want to send the card to, ie, persons name, address, zip, state, city etc. once the continue button is pressed this information needs to pass along with the order.
