Create a useable online database

We need a database built that does many different things. It should be searchable and able to add many different features over time. The coding must be clean and lightweight. This is a project that will be ongoing for the right person or team. We will give you certain tasks to do, and you must be able to do them within a specified time frame. Over time, we will add more tasks that may increase in complexity, but once you establish trust via your submitted work, timeliness and communication, the time deadlines may be extended. The first part of the project is a introductory phase: you will complete the assigned tasks, and it will give us an idea of your abilities and timeliness. You will only have 24 hours to complete the introductory phase of the project. Here's what we need: • You must construct an online database that consists of 21 different fields (Table 1) • You must also add a table that has 6 different fields (Table 2) • You must create a web form that takes information from what is entered, and disperses this information into the appropriate fields in the other 2 tables • You must test that this basic form structure works, host it on your own server, and allow us to test this new "program." • Finally, you must be able to export this "program," and instruct us how to install it from scratch on our own server We WILL NOT provide any testing environment for you; you must have this at your disposal. Furthermore, the testing environment must be something that we can access via the internet; Screen captures and screen sharing will not be sufficient for us to test what you have published. To apply, let us know what types of online databases you have created, the backend environment for those DBs, and how long it took for you to create the finished product. To vault to the top of our list, functional demos should be submitted for our review. This could be a site that is active currently, or private projects that you have worked on that are still live via the internet. Any questions, please let us know.
