2 page logging system

I would like a way for an employee to add log entries every time an employee refuels a company vehicle. I want the fields he submits to be as follows: Date of Fueling Cost of Fuel Vehicle ID (Drop Down) Vehicle Odometer Reading Upload photo of receipt On the admin page I want the admin to have the ability to add new vehicles (for the drop down), then see all submitted entries in tabular form as a report. I would the pages written using MDBootstrap Pro for Angular. I have a 2 different digital Oceans Droplets setup up for this project, one as a webserver, the other as a MySQL server. These droplets have not been setup or configured. I will send the server credentials as well as the plugin files upon acceptance of the project.
