WooCommerce: Csv Table Matrix Pricing

My client sells made to measure products (Kitchen Doors), they can either be "standard" size doors. For which I'm using this plugin: [login to view URL] And that works fine. But I also need a "custom size" option, where 2 custom fields will appear (Width, Height), and the customer enters their custom sized requirements. I want to be able to set custom prices based on a CSV file, example of which is attached. There will be two user inputs which affect the price, the Height and the Width.
