Software / Web Form Development

We would like a web/cloud based design app/form built to generate basic design work. The idea is that a client can input basic text headings within pre-defined areas of an identification label (that we manufacture). The webpage / form would need to allow them to upload a logo to be placed on the label also. We currently have 4 basic layouts, essentially, the label looks like a table grid with areas for text. In an ideal world we would prefer to allow the customer to adjust the table boxes as well, just like you would in a Word document for instance.. In it's simplest state we would like for the customer to be able to create mockup layouts, adjust text headings and upload a graphic logo. Then have this info emailed to us and the customer in a basic form submission system.
