Build a Website

This is a fairly novice task for a beginner programmer and can be completed in couple hours. In need for a PHP developer for a simple data driven website using myql for a small project. Currently I have a user group (50+ users) that has around 8 attributes of which two attributes are known (user email and user zone). There are around 6 more attributes that are not known, I will add the user group data with the email and zone. Remaining data will have to be filled in by the user on a web form. Once the initial data is entered - I will need the ability for the site to email single user/ selected group / all of the users - with a link and instructions to update the record belonging to them. Time of last email sent and record modified date time should be also gathered / recorded. The access should have two levels - admin level for running reports or sorting through data collected and a user level (no authentication needed here) for updating the form link with details. The link emailed should show the form ready to enter data. Once the user updates the form with required info they should be able to click on save and view the updated record. The admin level will have ability to view / add /edit / delete /sort / query based on table fields and export displayed data to csv. I might be missing a few minor features but all in all this is the extent of the project. Please feel free to ask any questions. There is no need for graphics - just a white background with form in tables should be good. I have a few more small projects coming up and if you do quick / quality work - I will be a loyal customer. Collaboration and communication is key so work is done right the first time.
